Case Management

Our organization delivers a variety of wraparound services to provide access and advocacy for Vietnamese Americans. Our Case Management program is a collaborative process of assessment, planning, facilitation and discussion to meet the health, legal and social service needs of our community. Our case manager and dedicated staff are here to work with individuals and families apply for community services, such as food stamps, housing, Medicare, social security and referral assistance, and helps identify local resources and aide.

As a collaborative process, our case manager interviews and evaluates each client to assess their needs and to develop a proper action plan. We provide on-going follow-up, referrals to social service and health agencies, facilitate small workshops, and develop informational materials for educational purposes. We believe that engaging clients and encouraging them to take an active part in their own development is crucial to their empowerment, independence and success.
